Vessels sailing under the Cayman Islands country flag are required to have on board this flag as part of flag state requirements that derive from maritime regulations in the International Code of Signals and the International Safety Management (ISM) Code. These codes are published by the International Maritime Organization (IMO) as an ongoing effort to reinforce maritime safety procedures and communications to the international maritime community. Fleet management companies and ship owners are responsible to ensure that each vessel in their respective fleets have the appropriate flag signage onboard to be in compliance. This flag is used to identify the flag state a vessel is sailing under and to communicate with other vessels under the provisions of the International Code of Signals.

The Cayman Islands flag represents the three Islands in the Cayman Islands. The three stars on the shield represent three Islands, they include; Cayman Brac, Grand Cayman, and Little Cayman. The coat of arms of the Cayman Islands consists of a shield, a motto, and a crested helm. The base is navy blue with a Union Jack in the corner to the top-left, and it is situated at the top right in the middle. Green turtle is on the upper part of the shield that represents the history of seafaring. The gold lion seen on the side represents Britain. Behind the turtle, there is a pineapple gold that represents the ties with Jamaica. Printed at the bottom of the shield is the Cayman Islands motto that says “He hath founded it upon the seas." This slogan reminds the Cayman Islands people of their ties to the seas as well as to their Christian heritage. The red background implies that the flag can be used at sea while the dark blue background means the flag can be used on land.
